Northwest Family Services is collaborating with Clackamas County Division for Children, Youth, and Families and the Gladstone School District and in particular, the Gladstone Center for Children and Families to launch a pilot project which will connect families with the resources to stabilize their current situations.

Family Resource Coordinator

The Family Resource Coordinator (FRC) will provide support and assistance to help families with children (ages 0-8) in the Gladstone School District (GSD) including Jennings Lodge and Candy Lane Elementary Schools. The FRC will focus on early childhood success including pre-kindergarten readiness and reading by third grade. Any family in the GSD with a child in that age range will be able to receive services. The FRC will identify services that meet the immediate need of the referred family and find the supports to match those needs. The primary focus of the FRC is to connect the participants with resources including early childhood screens and assessments, medical homes, family advocates, home visitors, and counselors.

  • Screens
  • Assessments
  • Medical Homes
  • Family Advocates/Counselors/Home Visitors
  • Family Resource Coordinator will identify resource in the community
  • Family Resource Coordinator will follow-up with families and assist with problems that may arise after initial linkage has been made.

Referrals can be sent by email, fax, phone or in person. Families can also self-refer, they can call us directly for information or to set up an appointment.
